Web Publishing Service Request Forms
Web Services
The following services are provided by the office of the Web Coordinator. If you have questions concerning any of the forms in this section, please email collage@selu.edu.
Identify Your Content Contributor
Since Collage is permissions-based, units will need to notify the Web Site Coordinator of any changes they wish to make with regard to Content Contributors in their unit.
Complete the Template Choice Form to choose or update the design template for your unit's Web site. The Office of Public Information and the Web Site Coordinator will work with you to develop a header photo for your home page that reflects the personality and individuality of your unit.
Photo/Graphic Image Request Form
Complete the Photo/Graphic Image Request Form to request any photographs and/or graphic images you need. You may also attach your own photos to this form to have them enhanced and sized for you.
Request New Web Space in Collage
Use this form to request new Web space in Collage for a new department, unit or program. If requesting new Web space in Collage you will also need to fill out the Template Choice Form.

Special Programming Request Form
The Special Programming Request Form is used to request Web forms or other Web based programming needs you may have. Once completed, the form should be emailed to Dave Koch (jdkoch@selu.edu) in Administrative Computing so that he may review the request and assign it to the appropriate person. When you submit the request via email please be sure to cc your unit head.
Use this form to request Web space on www2.selu.edu or www3.selu.edu and/or to create a streaming media account for Faculty and Staff only. Examples of non-Collage sites are faculty Web pages and student/staff organizations.
Change Your FTP (webserver) Password
Use this form to change your non-Collage Web space password.
Request a Short URL (Redirect)
Use this form to create a short URL for your site in order to simplify your Web address for the purposes of publications, business cards, and other literature.
