Disturbing Activity or Behavior Form
This form should be utilized to report disturbing activity or behavior that affects the University community (on or off-campus and/or in or out-of-the-classroom). Reportable incidents include any behavior or disturbance that is potentially or actually harmful to the individual exhibiting the behavior and or to faculty, staff, or students at the University. This form is intended to document incidents involving students who present behaviors beyond ordinary.
Any emergency or act that is criminal in nature should be reported to University Police at 985.549.2222 immediately.
