Steps to Self-Identify
Faculty will not provide accommodations without proper notification from the Office of Disability Services. It is the student's responsibility to self-identify with the Office of Disability Services and to request accommodations each semester.
Steps to Self-Identify as a Student with a Disability:
- Contact the Office of Disability Services (985-549-2247) to request information regarding policies and procedures for self-identification and accommodations.
- Complete the Student Intake Form (available to download at this link)
- Read and review the Office of Disability Services Policy and Guidebook (available to download at this link).
- Provide the Office of Disability Services with appropriate documentation of your disability.
- When the documentation is received by the Office of Disability Services and has been reviewed, an intake meeting will be scheduled to discuss accommodation needs.
- Once accommodations are determined, the student then fills out the Accommodation Request Form (available to download under the Forms tab) to receive letters to deliver to instructors. The accommodation letters will provide verification of adisability as well as the approved accommodation plan.
Documentation Guidelines
If appropriate, an evaluation must be conducted by a qualified professional.The documentation must include the name, address, phone number, and credentials of the professional who conducted the evaluation and must be on letterhead stationary.
The documentation must clearly state the diagnosis of the disability, a summary of the tests used in making the diagnosis as well as the resulting scores.
The documentation must address the nature and extent of the disability, the recommended accommodations, and describe the disability in behavioral terms in relation to the specific accommodations sought.
A student seeking accommodations is required to provide medical documentation that will substantiate the claim of disability. A licensed professional qualified in the area of disability must provide documentation on his/her office letterhead. Documentation must have been completed no more than three years prior to the date that the student submits a request for accommodations. The documentation should address the nature as well as the recommended accommodations, and should describe how the specific disability impacts functioning in the academic setting. The documentation should also address the specific diagnosis, the tests used in making the diagnosis, and when appropriate, scores.
